The Wisconsin Department of Health Services (DHS) wants to make sure every parent or caregiver of a child with a delay or disability knows about the benefits of early intervention. To do this, DHS is working on a public outreach campaign that will create materials and other resources to improve and expand the reach of the Birth to 3 Program.
Take the survey
Parents, caregivers, advocates, and others who help care for children with delays or disabilities are invited to take the Birth to 3 Program Family Outreach Survey. The survey is available in English, Spanish, and Hmong, and will take about 10-15 minutes to complete.
Feedback collected in this survey will help DHS understand how to effectively reach parents and caregivers across Wisconsin with timely and transparent information about the Birth to 3 Program. Please submit your thoughts by January 8, 2021.
